In November 2025, the FDA announced it would remove the "black box" warning — its most serious safety label — from menopausal hormone therapy products containing estrogen. The labeling changes were formally approved in February 2026, removing risk statements related to cardiovascular disease, breast cancer, and probable dementia from the boxed warning across estrogen-alone, estrogen-plus-progestin, and progestogen-alone products.
FDA Commissioner Marty Makary and HHS Secretary Robert F. Kennedy Jr. framed the decision as correcting decades of "exaggerated" risk based on "outdated science." The case for removal was strongest, and least controversial, for one specific product type: low-dose vaginal estrogen, which stays largely local and doesn't meaningfully raise blood estrogen levels — a growing body of research, including a 2025 study of nearly 57,000 postmenopausal women, supports that this form carries minimal systemic risk.
Why This Wasn't Really About Lower Risk
Here's what's worth sitting with: the decision applied the same warning removal to systemic estrogen — pills, patches, and creams that do reach the bloodstream — without distinguishing them from vaginal estrogen's much stronger safety case. No new randomized clinical trial emerged in 2025 showing that systemic estrogen is safer than the Women's Health Initiative (WHI) data already showed. The underlying risk data didn't change. What changed was how the FDA chose to weigh and present it.
The process itself drew scrutiny for the same reason: this decision came out of a two-hour expert panel convened in July 2025, not the full FDA Advisory Committee process — with independent review, public engagement, and a formal vote — that decisions of this scale typically go through. The American College of Obstetricians and Gynecologists and The Menopause Society both supported removing the warning specifically for low-dose vaginal estrogen, while maintaining that systemic estrogen still carries real risks requiring individualized discussion with a doctor — a distinction the blanket label change didn't preserve.
None of this means hormone therapy is unsafe or that the warning removal was baseless for every product it covered. What the Research Actually Shows
The real WHI findings are more specific — and more useful — than a blanket "estrogen is risky" or "estrogen is safe" headline:
- Estrogen plus progestin (used in women who still have a uterus, since progestin protects against unopposed estrogen's effect on the uterine lining) significantly increased breast cancer incidence in the WHI randomized trial, with elevated risk persisting for more than a decade after women stopped taking it.
- Estrogen alone (used only in women without a uterus, since there's no endometrium left to protect) actually showed a reduced breast cancer incidence in the same WHI trial structure — a genuinely different risk profile from combined therapy. It still carries its own separate, real risks: increased stroke and blood clot incidence.
- Unopposed estrogen in a woman with a uterus — meaning estrogen given without progestin — raises endometrial cancer risk substantially, which is precisely why progestin is included for anyone who hasn't had a hysterectomy.
The honest takeaway: which formulation you're on, your age at initiation, and your individual risk factors change the risk calculation meaningfully. A single warning label was always too blunt an instrument to capture that — and removing it doesn't make the underlying complexity disappear.
The Core Problem With ANY Added Hormone
Your body's own hormone production runs on a feedback loop — glands sense circulating hormone levels and adjust output accordingly, staying within a functional range. A prescribed hormone dose doesn't work that way. It's a fixed external input, and your body has to figure out what to do with it — metabolizing it through the liver, adjusting receptor sensitivity, and managing whatever level actually ends up in your bloodstream, which can vary by formulation, absorption, and individual metabolism even at an identical prescribed dose.
When that input runs consistently higher than what a person's system is equipped to handle, the result isn't always dramatic — sometimes it's the milder side effects associated with HRT (bloating, mood changes, breast tenderness), and sometimes it compounds into the more serious risks covered above. This is exactly why hormone therapy isn't a "set it and forget it" treatment.
Why Monitoring and Dosing Matter So Much
Getting a hormone dose right isn't a one-time decision — it typically requires bloodwork, symptom tracking, and dose adjustments over time as a person's body responds. That monitoring is also where real-world access problems show up:
- Follow-up bloodwork and specialist visits aren't always covered comprehensively by insurance, and out-of-pocket costs can lead people to skip monitoring they'd otherwise get.
- Without regular monitoring, a dose that was appropriate at the start can end up too high or too low for months before anyone notices.
- Compounded or pellet-based formulations, common in some clinics, are harder to titrate precisely than standardized pharmaceutical doses, which can make consistent monitoring even more important — and even easier to skip.
The Medspa Problem
A growing share of hormone therapy — particularly pellet implants and compounded formulations — is administered outside a traditional endocrinology or gynecology practice, at medical spas and wellness clinics with less consistent follow-up infrastructure. Medical societies including The Menopause Society have specifically raised concerns about compounded and pellet-based hormone therapy administered without the same individualized dosing and monitoring standards used in mainstream clinical practice, including documented cases of supraphysiologic (above-normal) hormone levels in patients using pellet therapy.
Convenience and accessibility are real advantages of this care model — but they come with a real tradeoff in oversight, and that tradeoff is worth knowing about before choosing where to start hormone therapy.
What Happens When You Stop
Discontinuing hormone therapy — whether by choice, doctor's recommendation, or running into an access barrier — commonly brings a withdrawal period: a rebound of the same vasomotor and mood symptoms the therapy was managing, sometimes more intensely for a period before settling.
